I am looking for any copies of applications or outlines of paperwork that is needed for a med marijuana grower's/provider's license. Thinking about getting into it in my state once it passes, which it just did or has one more vote to go, or when I go to grad school out of state applying for one in another state. Any info is helpful.

Currently, their site describes the Michigan Medical Marihuana Program, and provides general information and a FAQ about the program. The MDCH's website now also contains application forms for the Michigan Medical Marihuana Program.
If your medical condition is not on the list of legally qualifying conditions, please fill out a petition form and submit it to the MDCH.
